Webinar recording: The YTP - Transitioning Youth into Adult Services

This is the fifth webinar in the INTERVENTIONS IN PRACTICE webinar series, highlighting evidence-informed interventions implemented as part of the Systems Improvement through Service Collaboratives (SISC) initiative.

This webinar was developed by the Kingston, Frontenac, Lennox & Addington Service Collaborative.Webinar participants:

  • learned about a newly developed, evidence-based protocol to provide youth with a consistent and successful transition from youth services into appropriate adult services;
  • learned about the key elements of the Youth Transitioning Protocol (YTP) and Youth Toolkit and explored the ways in which the YTP may be used with the people you serve; and
  • heard about the lessons learned in implementing and sustaining a collaborative planning process; the challenges, the successes, the impacts and planning for the future in an ever-changing service landscape.

Target audience:

  • Agencies and partners supporting youth, particularly youth with mental health and addictions challenges;
  • Mental health and addictions agencies;
  • Stakeholders and partners with an interest in the Systems Improvement through Service Collaboratives initiative.
